Our mission is to ensure that each patient has the support necessary to assist them in reaching their goals, including ventilator management and weaning. Our staff will provide unique tools to assist in the success of the program, including educational materials and specialized equipment as necessary. Education will include lung disease education, proper medication usage, and breathing retraining.

Program Objectives:

• Coordinated effort and establishment of working communication between all involved parties, including hospital, practitioner, skilled nursing facility, & home health agency
• Consistent education for patients and families using shared materials & key learning components, including management of medications, diet, exercise, and heart-healthy lifestyle choices
• Successful transition to an outpatient pulmonary program
• Post-discharge follow up
• Interdisciplinary approach to disease management provided by our knowledgeable staff with enhanced education & training
